We are seeking a Family Nurse Practitioner to provide evidence-based Primary Care to our patients at our Health Center in White Plains Road, Bronx.

The organization provides a wide range of comprehensive, coordinated primary medical, oral, and mental health care and related diagnostic, pharmacy, social support, and enabling services for medically underserved/uninsured residents.

BCHN’s mission is to improve the health of people and families in the Bronx; provide access to affordable, quality health care, especially for uninsured persons; promote disease prevention, early treatment and healthful lifestyles; and obtain financial and other resources for programs and services. BCHN’s service area includes the entire borough of the Bronx, including all 25 zip codes. Within this area, Health Center sites focus on the low-income and underserved populations.

About the Role: The Family Nurse Practitioner (NP) is responsible for providing high quality care to adult and pediatric patients and their families. The NP will work closely with a team of professionals to provide a comprehensive patient-centered plan of care to achieve expected outcomes.

  • Provides primary medical care to patients and manages acute and chronic conditions.
  • Conduct age-appropriate assessments of patients, including history taking and physical examination in accordance with current standards of professional practice, regulatory requirements and agency policies and procedures.
  • Perform assessment/data collection in an ongoing systematic manner, focusing on physiological, psychosocial, cultural, spiritual, and cognitive status, patient/family education, family involvement, and patient advocacy.
  • Conduct age-appropriate assessments of patients, including history taking and physical examination in accordance with current standards of professional practice, regulatory requirements and agency policies and procedures.
  • Works continually to assure deliverance of quality clinical care in a safe, efficient, competent, and effective manner.
  • Refer to other clinicians as clinically appropriate.
  • Prepares records and reports as required in support of services rendered in accordance with established procedures.
  • Maintains accurate and detailed medical records about each patient, including observations during examinations (via EMR)
  • Administers or prescribes treatments and medications.

  • Promotes health by advising patients concerning diet, hygiene, and methods for prevention of disease.
  • Writes orders to inoculate and vaccinate patients to immunize patients from communicable diseases.
  • Collaborate with physicians and other clinicians to assure effective patient care consistent with quality indicators for patients cared for in the designated service.
  • Responsible for maintaining confidentiality of all patients, clients, proprietary, and protected information.
  • Participate in patient care conferences, professional staff conferences, and other appropriate professional activities related to his/her assigned by the CMO.
  • Perform other duties as requested by management.

Qualifications:

Master’s Degree graduate of an accredited Nurse Practitioner program

  • New York State Licensed Certification and Licensure as a Nurse Practitioner

Current certification in Basic Life Support (BLS) through the American Heart Association (AHA) or willing to obtain within

  • Board Certified

Current active DEA license

Relevant experience strongly Preferred.

Special Skills and Knowledge:

  • Experience working in community-based health care settings is also preferred.
  • Experience/An understanding of EMRs
  • The ability to demonstrate teamwork and effective interpersonal relations skills
  • Excellent written/oral communication and computer skills
  • Ability to foster and maintain a positive working relationship in a multidisciplinary team and clients.
  • Great bedside manners. Up-to-date knowledge of the latest technologies and treatments.

Salary: Exact compensation based on skills; experience $125,000 – 140,000 annually
