Position Summary:
We are seeking a compassionate and dedicated Clinical Assessment and Eligibility (CAE) Registered Nurse (RN) to join our interdisciplinary Home Care team. In this role, you will contribute to the development and integration of comprehensive service plans while conducting on-site assessments and clinical evaluations for individuals in our service area.
Essential Duties and Responsibilities:
Ensure compliance with all relevant state, federal, and agency regulations, policies, and procedures related to Clinical Assessment & Eligibility (CAE) and Home Care programs.
Conduct assessments for consumers, including MassHealth and Non-MassHealth recipients and applicants, to evaluate clinical status for community-based and nursing facility options in accordance with Massachusetts regulations.
Perform thorough initial on-site health assessments to identify personal care, behavioral health, and home health aide needs, developing tailored care plans while maintaining a reassessment caseload.
Ensure compliance with Preadmission Screening and Resident Review (PASRR) for assigned Community or CAE-related screenings and assessments.
Review and complete Pre-Admission nursing facility screening requests from partner hospitals and rehabilitation facilities, providing timely and appropriate clinical decisions.
Collaborate with the Interdisciplinary Team (IDT) to review Home Care/CAE consumers and make informed recommendations.
Work closely with Care Managers as a clinical consultant, assessing consumer status and ensuring care plans remain appropriate as conditions change.
Build and maintain strong relationships with service providers, hospitals, and community organizations to stay informed about available resources.
Educate consumers and caregivers about care plans, advocating for their needs and goals within the RN's scope of practice.
Ensure accurate and confidential completion of required documentation, including Comprehensive Data Set (CDS) assessments and MassHealth Level of Care Clinical Notifications, adhering to submission deadlines.
Assist in preparing for and representing appeals in the Fair Hearing Process at the Board of Hearings.
Attend all required staff meetings, in-services, and relevant training programs.
Perform additional related duties or special assignments as requested by the Nurse Manager or Director of Home Care.
Required Knowledge, Skills, and Abilities:
Bachelor of Science in Nursing or an Associate of Science in Nursing with at least one year of clinical experience in a community or long-term care setting; or a diploma RN with two years of community-based nursing experience or direct service in long-term care.
Experience in community, home health, case management, or nursing facility care is preferred.
Knowledge of the aging process, mental health, and developmentally challenged populations is a plus.
Understanding and acceptance of the multifaceted role of the Registered Nurse, including conducting on-site assessments, developing care plans, performing CAE screenings, and consulting with Care Managers.
Ability to work as part of an interdisciplinary team to assist nursing facility residents/consumers in overcoming barriers to discharge by implementing tailored care plans.
Certifications, Licenses, Registrations:
Valid driver's license with a satisfactory driving record.
Successful completion of background checks, including CORI, SORI, SAMs GSA, MassHealth Exclusion List, and OIG checks.
Current and valid MA RN license.
