Previous experience in home health is preferred
Current and unrestricted Virginia Physical Therapy license

Job Description:

Join our dedicated team as a Physical Therapist and make a meaningful impact in the lives of patients dealing with injuries and chronic health conditions. In this role, you will help patients regain their mobility, manage their pain, and enhance their overall quality of life.

Key Responsibilities:

Utilize a company vehicle to travel to patient homes and provide exceptional therapy services in line with their individualized care plans.
Observe and monitor patient progress during treatments, ensuring effective communication with patients and their families.
Act as a liaison among patients, their families, and our clinical team to facilitate seamless care coordination.
Provide hands-on care and patient education, empowering individuals to take charge of their health.
Document all visits thoroughly using Kinnser.net, our accessible point-of-care system, ensuring accurate records are maintained.
Familiarity with OASIS E and adherence to CMS guidelines is essential.
Compensation:

Competitive visit rate ranging from $100 to $130 per visit (approximately 45 minutes in duration).
