An established, mission-driven healthcare organization is seeking a Licensed Professional Counselor (or similarly credentialed professional) to join their multidisciplinary team. In this role, you’ll guide patients through life’s challenges, support emotional wellness, and help shape integrated care solutions that make a meaningful difference.
What You’ll Do
As a valued member of a collaborative care team, you’ll deliver high-quality behavioral health services while working closely with providers to ensure coordinated, patient-centered care.
Therapeutic Interventions
Provide evidence-based therapy focused on behavior modification, coping strategies, and emotional health.
Assist patients in processing trauma, building resilience, and overcoming barriers to wellness.
Integrated Care Collaboration
Partner with physicians, care managers, and primary care staff to develop and maintain comprehensive care plans.
Serve as a behavioral health expert, offering insights on patient needs and emerging trends.
Patient and Community Support
Conduct screenings and assessments to determine appropriate therapeutic approaches.
Support patients and families in accessing community resources, referrals, and specialized care when needed.
Respond to crisis situations, advocating for timely services and patient support.
Documentation and Confidentiality
Maintain thorough, accurate records of patient care in the EMR system.
Uphold strict confidentiality and adhere to all professional ethics and compliance standards.
What You’ll Bring
We’re seeking a compassionate, collaborative professional dedicated to improving mental health outcomes.
Qualifications
Licensed as LPC, LLPC, LMSW, LMHC, or CADC with current state credentials.
Master’s degree in psychology, counseling, social work, or a related field.
Experience
Preferred: Experience working with diverse age groups (adolescents to geriatrics).
Background in primary care or integrated health settings is a plus.
Knowledge and Skills
Strong understanding of behavioral health principles, therapeutic methods, and cultural factors influencing mental health.
Proficiency in diagnosis, treatment planning, and rehabilitation processes.
Familiarity with electronic health records (EHR).
Excellent interpersonal skills with a patient-centered approach.
Ability to lift to 40 pounds as needed.
